• Assisted/Supportive Living Referrals
• Referrals to home health care agencies
• Referrals to Hospice
• Referrals to equipment companies
• Referrals to Alternative for the Older Adult/Department of Rehab Services
• Elder and child abuse referrals
• Completion of Living Wills, Health Care Power of Attorneys, and wallet cards
• OSF Care Decisions
• Emergency Room visits
• O.B. routine referrals and visits
• Intensive Care Unit visits
• Referrals for outpatient psychiatric assistance
• Referrals for prescription assistance
• Financial evaluations for assistance
